What is melasma?

Melasma is a common skin condition characterized by dark, discolored patches on the skin, typically on the face. It often affects the upper lip.

What causes melasma?

The exact cause of melasma is unknown, but it is believed to be triggered by hormonal changes, sun exposure & genetics. It is more common in women, especially during pregnancy (often referred to as “the mask of pregnancy”). Melasma is more common in ethnic skin types.

 

Here are 3 simple steps to fix melasma pigmentation!

  1. Limiting flare factors. The number one flare factor is UV radiation, namely UVB, UVA & visible light. Using sunscreens in the correct dose & frequency is the foundation of melasma management. If you don’t get this right, all else will fail. 
  2. Reducing pigment production. This is achieved by pigment inhibitors & vascular modulators. The ideal pigment corrector depends on your skin type, pigment depth, & skin sensitivity. Vascular modulators are used if you have blood vessels that feed your pigment cells. 
  3. Pigment removal. This can be a combination of lasers & or novel chemical peels. The type of peel (or laser) depends on the depth of pigmentation.

How long does it take to see results from melasma treatments?

The time to see results can vary depending on the treatment method and individual skin response. Topical treatments may take several weeks to months, while procedures like chemical peels or laser therapy show quicker results, often within a week or two. 

Regardless of treatment, the best results are seen at 3 to 5 months after initiation of therapy.

How do I know if I have melasma?

Most cases are easily diagnosed by a dermatologist. Melasma is based upon clinical features including distribution as well as dermatoscopic features (magnified findings to ascertain pigment depth). 

A blood test is not required & a biopsy of skin is rarely required.

What pigment correctors do we prescribe for melasma?

Pigment correctors block excessive pigment production. Entry level pigment correctors include botanicals, azelaic acid, vitamin B, arbutin, &  kojic acid.

Medium to high strength pigment correctors include vitamin C, hydroquinone & cysteamine. 

Our clinicians will often combine pigment correctors in various bases & concentrations for a customized approach to skin pigmentation. It depends on factors such as-

What are the best lasers for melasma?

The Melasma Clinic uses over a dozen pigment lasers to treat melasma & skin pigmentation. 

Pico lasers work faster in clearing melasma pigment, & they are more effective as compared to older fractional & Q switch lasers. 

For stable melasma, we use thulium fractional lasers to remodel the deeper layers of skin. This can place melasma into remission. Vascular lasers can also treat the blood vessels that feed the pigment cells.

Lasers are more comfortable than chemical peels as they do not have any downtime. Results are dependent on the settings of these devices. 

What are the best chemical peels for melasma?

The Melasma Clinic uses over 6 chemical peels to address melasma pigmentation. The ideal peel type depends on the depth of pigmentation, your skin type, downtime & expectations. We perform common peels such as Cosmelan, Dermamelan as well as novel Vi Precision Plus peels, AHA & retinoic acid peels. In 95% of cases we prefer the Vi Precision Plus peel because it has a similar efficacy to Cosmelan with much less recovery.

What is best, chemical peels or lasers for melasma?

Peels have a faster onset of action compared to pico lasers, however they have more downtime. Pico lasers, used in the correct settings provide a better overall melasma clearance with the added benefit of skin rejuvenation. The Melasma Clinics employ both treatments, depending on your skin types, melasma pigment depth, downtime & previous melasma treatments.

Lasers are more comfortable compared to chemical peels & can be used in patients with sensitive skin. Additionally the post treatment skin care with pico lasers is far easier compared to the Cosmelan 2 system.

Is the Cosmelan Peel overrated?

Don’t get us wrong, we do employ the Cosmelan peel for melasma (& pigmentation). It is effective in over 60% of cases, however it does have some cons, namely skin irritation that can last up to 2 weeks. This peeling system is an easy to use, step by step kit, hence it is popular with many clinics.

The Dermamelan peel is stronger, whilst the Vi Precision peel is nearly as good as the Cosmelan, but it has much less downtime.

Cosmelan Pros: good results in a short period of time, easy to follow kit with instructions.

Cosmelan Cons: long recovery time, no long term game plan for melasma.

Will all cases of melasma respond to treatment?

80-85% of melasma will respond to a combination of medical therapy, lasers & novel chemical peels. 

High response rates: Post pregnancy melasma, superficial pigmentation, indoor workers, rebound melasma (eg. flare ups after lasers). 

Lower response rates: deep or dermal melasma, poor sunscreen use. Outdoor workers, cooks (heat), mature ethnic skin types. 

*Response is defined scientifically as a reduction of over 50% in the MASI or melasma area severity index. It is objective & not subjective.

Why is tinted sunscreen essential for melasma prone patients?

The number one flare up of melasma pigment is UV radiation, not only UVA, UVB (blocked by most sunscreens), but also visible light. Visible light can only be blocked with tinted sunscreens (iron oxide).

Can heat flare up melasma pigmentation?

100% yes to that! Heat sources radiate IR or infrared light, this spectrum of light is notorious as a flare up factor for melasma pigmentation. Sources of IR include-

  • Cooking, especially open fire/gas stoves. This is commonly seen in cooks. 
  • Saunas
  • Fireplaces

Blocking IR heat is hard. Sunscreen does not block this spectrum. The best way to attenuate IR is via a heat shield like this one from Kelco Supply.

What is the cost for melasma treatment?

Costs vary depending on the pathway you choose. 

  • Chemical peels start from $920 per session.

The number one mistake

The most common misdirection made by the majority of melasma sufferers is underestimating the importance of photoprotection, namely sunscreens. People will spend hundreds of dollars, often thousands with lasers, pico, Cosmelan peels, skin care & more, only to underutilize simple sunscreen. The fact remains that photoprotection is the cornerstone of melasma management. It is not hard. You brush your teeth at least twice a day, so you should use SPF at least twice a day. Fact: it takes you longer to brush your teeth than to apply or reapply sunscreen. 

If you are going to drop some coin in treating melasma, start off with sunscreen. Forget Cyspera, TriLuma & the garbage of Drunk Elephant, get some sunscreen. It doesn’t need to be super expensive like UltraViolette, or Naked Sundays, just find a brand that you like to use, & use it. Make sure it is tinted. I do like the above brands for their color matching, especially in ethic or darker skin types (think inclusive). Other brands include Ilia, & SuperGoop. If Korean brands tickle your skin palate, go for it. Beauty of Joseon is recommended. 

Cosmelan is overrated

Before the negatives, here is the disclaimer- Cosmelan, it is still one of the better novel peels out there & we have been using this melasma peel for the past 6 years. The upside is that it is a very easy peel to ‘upsell’ by cosmetic clinics & beauticians as everything is in the kit. Essentially an idiot proof peel with step-by-step instructions. Ease of use? Anyone who can read the instructions can perform this peel. So easy, a dermatologist can do it. The second positive is the fast results. Cosmelan results are faster than pico lasers or other peels. 

Downside? Huge downtime that is somewhat unpredictable, namely ranging from 5 to 18 plus days. It doesn’t work for dermal melasma (in all fairness, nothing much works) & it doesn’t address long term issues, namely recurrence. The kit is shortsighted in the approach, with the upsell of Cosmelan 2 as maintenance. 

Pico lasers are great, but it is in the settings

Pico lasers are like a pair of high grade Japanese steel scissors. GIve it to an apprentice & the results are mediocre, give it to an expert & the results can be phenomenal. It is all in the settings & how one uses the tool of choice. We use one of two pico lasers, the Picoway & the Picosure Pro, in our own bespoke settings. Each session takes between 30 seconds to 3 minutes to perform.

In it for the long term (remission & ‘cure’)

95% of melasma patients will not get this approach as most are focused on putting out ‘fires’ namely pigment flare ups, rather than focusing on a long term cure or at least, long term remission. The 5% that understand the aim of treating cellular senescence-photoaging will generally do better (it is NOT a guarantee, but it is the best way to achieve pigment clearances). Here is the concept-

  • Melasma is more than just increased pigment from melanocytes (pigment cells) 
  • One of the causes is cellular senescence, ie. getting old. Old cells in the dermis cross talk with other cells. Their messages (cytokine profile ie. how cells communicate with each other) age & change with time.
  • Treating cellular senescence effectively (it goes well beyond DIY skin care) not only improves photoaging, it can place melasma into remission for variable periods. 

This is how we address cellular aging-

  1. More powerful lasers, such a thulium 1927 in a very low setting. Low power, low density, many sessions. 6-12 sessions conducted every 4 to 10 weeks, spanning 18 to 36 months. Flares (mild) are not uncommon. Thulium remodels skin in a different way as compared to pico lasers. 
  2. RFM (think Sylfirm X or Potenza); low power, split pulse. Many sessions over 1-2 years.
  3. HIFU superficially; again this remodels the dermal layer of skin. 
  4. Adjunctive topicals; including sirolimus (rapamycin), topical high strength prescription retinoids (forget fluffy retinol).
  5. Dermal fillers; including Radiesse, Sculptra, PCL & more. Placed in hyper diluted concentrations in the low dermis- fat layers, this can remodel the basement membrane & dermis. Profhilo can do the same if you are risk averse, Rejuran can have a similar effect, however it works as a scaffold builder, rather than a remodeling agent.
